LPN- Licensed Practical Nurse - Per-Diem - All Shifts

Details: AristaCare at Whiting, a 180-bed premier Sub-Acute nursing and rehabilitation center located in the beautiful and serene Whiting, NJ (5 minutes from Toms River) serves 3 distinct populations including sub-acute & intense cardiac rehab, long-term care dementia and traditional long term care residents. We are currently looking for qualified per-diem Licensed Practical Nurses (LPN) for floor nurse positions for all shifts to join our wonderful dedicated team and to work in our newly renovated facility. As a per-diem employee you will have the flexibility to select any available shifts that work for you.JOB SUMMARYThe Licensed Practical Nurse is responsible for providing direct skilled client care under the supervision of a registered nurse.  Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes.ESSENTIAL DUTIES AND RESPONSIBILITIES Provide direct patient care as defined under the state nurse practice act using sound judgment and critical thinking. Implement plan of care and treatment initiated by the registered nurse. Provide accurate, detailed and timely documentation consistent with the plan of care and the nursing process. Conduct ongoing assessments. Participate in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers. Observe client condition and accurately reports to the registered nurse. Assist the registered nurse with client discharge or transfer. Assess and provide client and family/caregiver education and information pertinent to diagnosis. Prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous and intramuscular medications.  Observe, report and record adverse reactions to medications and/or treatment to the registered nurse. Recognize the need for and institute emergency measures in a rapid and appropriate manner, when indicated. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e., PT, OT, ST, Dietician) to the Director of Nursing or registered nurse supervisor. Perform wound care. Perform all skills, treatments and procedures competently in accordance to the scope of practice as defined in the Nurse Practice Act in the state of clinical practice and adheres to the policies, procedures and protocols of the agency. Follow principles of infection control and Universal Precautions. Identify and participate in performance improvement activities. Use equipment properly and in a safe manner.
