Details: Note:  Please read the complete description below before applying for this job.  Complete DescriptionProvena Sacred Heart Home, located in Avilla, Indiana, provides a full continuum of high-quality care that includes rehabilitation services, intermediate and skilled nursing care, assisted living and independent living for older adults. We currently are seeking Staff Nurses to join our JOB DESCRIPTIONJOB DESCRIPTION POSITION: Staff NurseJOB SUMMARY:The purpose of this position is to provide direct, professional, quality nursing care to residents assigned this position.  This is done in accordance with State/Federal guidelines and the facility policies.  It also involves overseeing direct care rendered by the C.N.A.'s.QUALIFICATIONS:1.       Must have an current licensure as a L.P.N./R.N. in the state of Indiana.2.       Must possess the ability to make independent decisions when circumstances warrant such action in a timely fashion.3.       Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.4.       Must be a minimum of twenty (20) years of age.5.       Must possess leadership and supervisory ability and the willingness to work harmoniously with other department personnel.6.       Must be willing to seek out new methods and principles and be willing to incorporate them into existing training practices.7.       Must possess the ability to plan and develop new programs.8.       Must have patience, tact, a cheerful disposition and enthusiasm.9.       Must be able to relate information concerning a resident's condition.PHYSICAL REQUIREMENTS:1.       Must be able to move intermittently throughout the work day.2.       Must be able to cope with the mental and emotional stress ofthe position.3.       Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of the position can be fully met.4.       Must function independently and have flexibility, personal integrity and the ability to work effectively with residents, personnel, support agencies and outside agencies.5.       Must be in good general health and demonstrate emotional stability.6.       Must be willing to perform tasks that may involve exposure to the residents' blood/body fluids.7.       Must be able to lift, push, pull and move a minimum of 50 pounds.8.       Must be able to assist in the evacuation of residents.POTENTIAL EXPOSURE TO BLOODBORNE PATHOGENSThis position is a Category I position.  Tasks may involve exposure to blood, body fluid or tissue, requiring the use of personal protective equipment.  Gloves should be worn for any contact with blood or body fluids/tissue.  Gowns and masks or eye coverings may be needed if it is likely that soiling of clothing or splattering of blood may occur.POTENTIAL EXPOSURE TO WORKPLACE VIOLENCE:  Category 4 - HIGH RISKTasks that have a high possibility of coming into contact with a violent person or situation but employment may require performing unplanned category 4 tasks.  The normal work routine involves a very high possibility of coming into contact with a violent person or situation at least once per shift, if not more often however, contact or potential contact at some time may be required as a condition of employment.ENVIRONMENTAL CONDITIONS:1.       Sits, stands, lifts, and moves intermittently during working hours.2.       Works beyond normal working hours and on weekends andholidays when necessary, as well as in other positions as needed.3.       May work flexible hours.4.       Is subject to frequent interruptions.5.       Attends and participates in continuing educational programs.ACCOUNTABILITY: Is accountable to the Director of Nursing or Designee.ESSENTIAL JOB FUNCTIONS:1.       To know and follow all rules of State/Federal guidelines and Provena Sacred Heart Home policies.  Medicare and Medicaid regulations, State and Federal rules.  Any and all violations of above will be reported to your supervisor.2.       Receives and gives report to all staff when arriving and leaving the facility.  Shares all information concerning resident care necessary to see that continuity of care is provided.3.       Protects and maintains residents confidentiality by giving information only to those with a right to know.  Resident information will only be shared on private settings.4.       Documents all nursing care rendered, this includes medications, treatments assessments and other pertinent information. Documentation will be legible, accurate and complete.5.       Oversees each shift that C.N.A.'s  give appropriate care based on resident needs and sees that their documentation is legible, complete and accurate.6.       Assists with completion of M.D.S.  and care plan development as required.  Follows care plans as written.7.       Provides professional nursing skills such as resident assessments, IV care, tracheostomy care, CPR, tube feedings, wound care, etc., as the resident requires.  Must maintain IV, Mantoux and CPR certifications.8.       Works on floor with C.N.A.'s to see that quality care is given to resident in a timely fashion.9.       Answers resident call lights and provides direct hands on care.10.     Works in role of C.N.A. whenever and wherever needed and does so graciously.11.     Administers medication to resident as per policy and notes resident responses.12.     Completes treatments as ordered by physician and notes resident responses.13.     Notifies physician of resident condition changes and seeks appropriate intervention for care.  Notes and processes orders in a timely fashion.14.     Keeps resident, resident sponsor or family informed of condition changes.  Answer questions as they arrive.15.     Makes rounds with physicians to see their residents.  The nurse will be prepared and conduct self in a professional manner.16.     At all times, deals professionally with residents, families, staff and  physicians.  Professionalism is evidenced in language (no cursing or slang use),  Additional InformationWeekend and/or Holiday Rotation may be required.Provena Health is committed to diversity.
